NEW CLIENT INTAKE FORMS 

    You can speed up your initial visit by completing our Intake Form before you come in. Simply click here to open and print out the forms. Fill them out and bring them with you to your first session so we don't have to take session time dealing with paperwork. Be sure to come a few minutes early the first time so we can get the rest of the information we'll need ready for your chart.

PAYMENT INFORMATION

    Payment for services rendered is expected at the end of your session. If you require special payment arrangements, please make those in advance of your session. We accept Visa, MasterCard, Discover and American Express, but we prefer cash, check or debit card due to the high service fees for credit cards.

    Some therapists at Aim Counseling Center take insurance for payment of your counseling services, and some do not, or operate on an adjustable fee based on your income. If you wish to make arrangements other than paying in full out of pocket for your session, please check with the office manager or your therapist to verify what the payment options are available in your particular situation. We will be happy to answer any questions you have about the best way to pay for services. There will be a $25 service fee for any returned checks.

    When you make your initial appointment, we will ask for a credit card to hold the appointment you make. Your card will not be charged unless you don't come for the appointment without notifying us. This is due to the 24 hour cancellation policy we have in effect. If you are unable to keep your appointment, feel free to call and reschedule or move it. If you fail do so within 24 hours of your appointment time, or    you simply don't show up, your card will be charged the standard session fee.

INSURANCE INFORMATION

    There are many different insurance carriers and many different plans within each carrier. Some therapists at Aim Counseling Center take insurance for payment of your counseling and others do not. There are things that may affect how much you have to pay - some plans require you pay a "co-payment" and others have a yearly deductible that must be met before the plan will pay. Unfortunately, it is almost impossible to determine exactly what any insurance plan will or won't pay for or cover until we actually receive an EOB (Explanation of Benefits), but we will do our best to authorize and confirm your plan in advance of your appointment. You may also investigate your status with your insurance company by calling the 800 number on the back of your card.

NOTICE TO ALL NEW CLIENTS USING INSURANCE

Effective April 15, 2008:

    It is now necessary for you to contact your insurance company in advance of your appointment time to obtain any AUTHORIZATION your insurance company may require to pay for your session.

    In order to determine if your particular insurance plan has this requirement, you will need to call the 800 number listed on your insurance card. If an authorization number is required, simply request one FOR THE PERSON ACTUALLY BEING SEEN in the appointment, and bring it with you to your first appointment.

    If your insurance company requires an authorization number and you do not get one, we cannot bill them, and THEY WILL NOT PAY FOR YOUR SESSION. 

    Please be advised that insurance companies are very difficult to work with, and often have very strict rules about authorizations. Frequently they will not “back date” an authorization if you wait until after your session to get the number.

    In the event an authorization number is needed you do not get an authorization, you will be financially responsible for the normal session fee of up to $175.00 per visit.

WHY YOU MAY NOT WANT TO USE YOUR INSURANCE BENEFITS TO PAY FOR YOUR SESSION:

    Before your insurance company will pay for your session, they will REQUIRE

    - information about your session - therefore you do NOT have 100% confidentiality
    - that you MUST be given a MENTAL ILLNESS DIAGNOSIS
